HEAL is a program that links community and clinical resources to prevent chronic disease by addressing social determinants of health and promoting policy, systems, and environmental changes.

Healthy Environments Active Living (HEAL) is a program within the state Department of Health and Human Services (DHHS). Formed in July 2013 (as EPICC) through CDC funding, it merged the Heart Disease and Stroke Prevention Program, the Diabetes Prevention and Control Program, the Physical Activity, Nutrition and Obesity Program, and a school health program. HEAL was restructured to address social determinants of health, advance health equity, and increase policy, systems and environment changes. Vision: All residents have equitable opportunities to lead healthy, informed, safe, and productive lives. Mission: Create community-clinical linkages and improve education, policy, built environment, and access to quality care in preventing and managing chronic diseases. Guiding Principles include Collaborative, Communication, Data-driven, Equitable, Innovative, Respect, and Work-life Balance. Overarching Goals are Healthy People, Healthy Communities, and an Equitable Society. HEAL emphasizes partnerships with staff, partners, and community organizations to improve population health outcomes and to support policy development and environmental changes that enable healthier choices and better access to care. The program also maintains data and reports resources, supports CHW state certification information, and promotes nutrition, physical activity, and healthy living while focusing on health equity and reducing chronic disease risk.

CHW Core Skills Training Program

CHW Core Skills Training Program provides core skills training for Community Health Workers (CHWs) as part of the HEAL program. The course will no longer be offered after April 2025. Future CHW Core Skills Training Courses will be offered through the Utah Community Health Workers Association (UCHWA). For certificates, contact Cassie Cowdell; information is also available via Joyce Kim and the UCHWA site.

TOP Star

TOP Star is a HEAL program to improve nutrition, physical activity, and breastfeeding environments in early childcare settings. It offers free online continuing education training (for endorsement), policy development assistance, menus, lesson plans, posters, and tools. TOP Star facilities can be endorsed, with guidance from local health department specialists.

Eat Well Utah is the statewide initiative to make healthy food choices more available and appealing wherever food is served or sold. It offers organization-level support through patron-desire assessments, technical assistance (nutritional analysis, menu planning, product placement), policy recommendations and educational materials, and promotion via a recognition system, with potential funding for implementation efforts.

CHW State Certification
